Founded in 1994, the firm was originally named Banowsky, Betz & Levine, P.C. after its three founding shareholders, Baxter W. Banowsky, Michael J. Betz and Scott D. Levine. The three lawyers had previously worked together at Bickel & Brewer. In 2005, Mike Betz left the firm to take the position of Deputy City Attorney with the City of Garland. As a result, the firm was renamed Banowsky & Levine, P.C. In 2022, Scott Levine also left the firm to take the position of Deputy City Attorney at the City of Garland, resulting in the change to the firm’s current name, Ax Injury Law.
